This Provider Search lists medical providers in PersonalCare’s provider network. This data is updated weekly for your convenience. While it is our goal to provide accurate information, our provider network is subject to change without notice.
It is important to know when you enroll in PersonalCare that services are provided through our delivery system, but continued participation of any one doctor, hospital or other provider cannot be guaranteed, even if a member is referred by his/her physician to a provider.
We strongly encourage you to call your provider of choice to confirm his/her participation in PersonalCare’s provider network.
Please Note:
- OSF St. Francis Medical Center and Children's Hospital of Illinois in Peoria are contracted with PersonalCare as tertiary providers. Therefore, members residing in Peoria, Tazewell and Woodford counties can access these providers only for emergency care.
- University of Chicago Medical Center and University of Chicago Physician Group are tertiary providers for PersonalCare HMO/POS members, therefore prior authorization is required for all services.

Weight Management Coverage
Effective July 1, 2008, PersonalCare will adopt a new approach to coverage for weight management surgery. All PersonalCare members who meet the applicable criteria and plan to have weight management surgery will need to have those surgical services performed by one of PersonalCare’s Bariatric Surgery Centers of Excellence in order to receive coverage. These services also require a prior authorization from PersonalCare and the ordering physician.

Level 3 of the PersonalCare Freedom PPO plan includes Coventry Health Care National Network (CHCNN) providers which are located outside of the PersonalCare, Group Health Plan, and CHC of Iowa service areas.
